General Setup Notes:
Support not provided. To setup and use this script on your hosting account you will need to:
View the readme in the downloaded "mattfm-perl-20030127.zip" file.
Follow the directions at the top of the "mattfm.cgi" in the zip download to edit the variables for the "my @referers" and "my $smtp" lines and to set any other form options.
Upload the "mattfm.cgi" to your hosting cgi-bin.
Chmod 755 the "mattfm.cgi".
Save the sample form to your computer and follow the instructions on the form.
Edit the following line in the sample form to the path with your domain.
